  Box A-

• First, you will mark New
• Then, mark the box Roof Mount
• For the question “were any structures built to support the system?” you will mark No
• Next, mark the boxes Solar Electric and Photovoltaic

         For the # of Kilowatts (you will find this in your contract)

• Lease contract you will find this on Page 3 under the System Description

• Cash contract you will find this on Page 2 where it says System Size

         For the # of Panels (you will find this in your contract)

• Lease contract it will be located on Page 3 in the System Description box

• Cash contract you will find this on Page 2 of your contract after the PV panels

 The next part is for whether you have a Cash or Lease contract.

      Cash Contract- 

• Mark the box Owned
• Gross installed cost you will fill in the amount you have paid on Page 3

      Lease Contract-

• Mark the box Lease

    -Name of System Owner is Lifestyle Energy

    -Address is 21 E Shaw Ave. Fresno

    -   Mark the box Lease Agreement (monthly rental amount)

            - On Page 4 of your contract you will find how much you pay monthly

            - Terms of Agreement: Start date will be the date you signed the contract

   -What happens at the end of the term? Check the Purchase box

Completion Date (date project usable) - this date will also be the date you signed the contract
